CHANGES IN THE MEDICAL HEALTH PLANS

Effective November 1, 2009, the following changes in the Medical Health Plan will take effect:

The 20/30/70 Personal Choice Plan ($20 for Doctors visits, $30 for Specialists and 70% covered out of network) will become the 25/35/70 Plan ($25 for Doctors’ visits, $35 for Specialists and 70% covered out of network).

The Keystone HMO Plan ($15 for Doctors’ visits, $20 for Specialists) will become $25 for Doctors’ visits and $35 for Specialists.

As in the past, there are no co-pays for inpatient hospital deductible and outpatient surgery deductible.

In addition, there is no change in emergency room co-pays.

Prescription Drugs which presently cost $10 for generic, $20 for Formulary and $35 for Non-Formulary for both the Personal Choice Plan and the Keystone HMO Plan will increase to $25 for Formulary and $40 for Non-Formulary.  The cost of generic prescription drugs will remain at $10.

The teachers medical contribution for Personal Choice remains at twelve and one-half percent (12.5%).  However, since the premium is expected to increase, the teachers’ actual cost will also increase.

The teachers’ medical contribution for Keystone HMO which currently is ten and one-half percent (10.5%) will increase to eleven percent (11%).

Medical premium deductions will begin with the pay of November 13, 2009.
